Re:  REFUND OF LAND TRANSFER TAX TO OHOSP PLANHOLDERS

We are writing further to your letter of June 19, 1989, in which you requested our comments on the consent wording that will be used to obtain a refund of land transfer tax to OHOSP planholders.  Please accept our apology for the delay in responding to your request.

The consent wording that is being used to access tax information is acceptable.  The only comments we received from the Department's operational group related to the absence of a social insurance number in Section 5, Certification.  Although the taxpayer's social insurance number is already entered on page 1 of the application form it was felt that by again entering the number on page 2, this would minimize any errors.

You will find attached a copy of page 2 of the application form showing the recommended change.

As you may have already gone to print with your form this change could be considered in the event the form is revised at a later date.
